How to get started
Thank you for your interest in joining Community Health Options Provider Network and partnering with us to make a meaningful impact in the health of Maine and surrounding communities!
As Maine's only locally based, not-for-profit health insurer, we are committed to expanding access to care for our Members. We welcome the opportunity to partner with all behavioral health, facilities, practices and ancillary services providers located in Maine and New Hampshire.
Step 1
Complete the "Request to Join Network" form above. It will be reviewed by our Contracting team.
Step 2
The contracting team will review for network adequacy. Please allow up to 90 days for network evaluation.
Step 3
If network need is determined, contracting team will send provider the required credentialing forms & documents.
Step 4
Once all required forms are received and both parties have agreed to the contract terms, the provider will proceed to the credentialing process.
National Network
We provide a nationwide provider network via our partnership with First Health® network. You can learn more about First Health's Provider Network here.
Provider Credentialing
We use the CAQH application as our credentialing application. To prevent delays in the credentialing process, please ensure the following:
- Authorize Community Health Options to access the practitioner's CAQH profile.
- Verify that all information in the CAQH application is accurate and up to date.
- Ensure all supporting documents are current and uploaded to the account.
- Confirm that the Provider's CAQH profile has been attested.
Please note: Practitioner credentialing begins once the Provider’s CHO Contract is executed, and the facility or practitioner data is loaded into our system.
